How to write a compound prescription
Please include the following information. We are happy to consult with you regarding your patient’s prescription if you have questions.
- Patient name
- Date of birth
- Best daytime phone number for patient
- Active drug name(s)
- Dose/Strength/Concentration
(i.e. Oral Tablet, Cream*, Suppository, etc.)
*Cream Preparations – We are happy to do the math per application; just indicate the dose and we will calculate the concentration.

- Quantity to dispense
- Number of refills authorized
- Contact information for Prescriber*, Address, Phone, NPI and DEA
*Prescriber Signature – Please provide an authentic prescriber signature for all controlled substances. Stamps and electronic signatures faxed in are not considered valid by the DEA.

Instructions
- Remove cap from end of syringe.
- Gently depress plunger so the rubber stopper moves one small line in the syringe to dispense the prescribed amount of gel on to gloved finger (0.1ML = one small line on the syringe).
- Spread gel evenly over hairless skin of inner ear pinna. Do not push gel down into ear canal.
- Rub in well.
- Replace storage cap on end of syringe.
Tips
- Storage: store at room temperature and out of reach of children
- Do not use a cotton swab to apply as the gel will adhere to the cotton and not the pet's skin.
- Do not use after expiration date
- If gel gets on your skin, wash area with soap and water.
- If build up of gel occurs in the ear, wipe away with a damp cotton ball

Oral Suspension/ Adapt-A-Cap Patient Instructions
- Replace white cap on prescription bottle (child resistant) with blue adapt-a-cap (NOT child resistant). Press down and twist clockwise until tight.
- Fit oral syringe tip into the blue cap orifice and invert assembly.
- Draw out the prescribed dose into the dispensing syringe by pulling back on the white plunger.
- Invert assembly again so it is right side up. Liquid will not drain out of syringe! Grasping the dispensing syringe by the barrel (NOT the plunger), quickly remove the syringe.
- Close snap cap to seal bottle and store as indicated. You may also store with the dispensing syringe inserted in the orifice, but be careful when the bottle is shaken.

- Remove white cap (child resistant) from prescription bottle using a counter- clockwise twisting motion with gentle downward pressure.
- Fit oral syringe TIP into the white port orifice and invert assembly.
- Draw out the prescribed dose into the dispensing syringe by pulling back on the white plunger.
- Invert assembly again so it is right side up. Liquid will not drain out of syringe! Grasping the dispensing syringe by the barrel (NOT the plunger), quickly remove the syringe.
- Administer dose to patient and replace white child resistant cap to seal bottle and store as indicated.
